Patient Services

High quality patient care and satisfaction is at the heart of our mission at Refuah, and we utilize a team approach to support our patients.

Refuah’s Patient Advocate is an anonymous willing ear for the community, who empowers patients by acting as a clinical liaison, resolving disputes, communicating on behalf of our patients, and keeping patients safe and well-informed. The result is that patients are given a voice on healthcare-related issues that matter most to them.

Concerns that can be addressed to the Patient Advocate include:

  • patient rights
  • matters of privacy
  • informed consent
  • patient representation
  • support and education of patients and their caregivers

We also have a Patient Services Division which helps navigate the system and guides patients to what services are available at Refuah and in the wider community. Our team of Case Managers work with individual patients to identify their needs and facilitate social services that may be helpful to them. Our Care Coordinators ensure continuity of care for our patients by serving patients who may have more complex medical needs and require outside consultations, referrals and medical equipment or medications. Once your appointment is made, please click the following link for our patient registration forms.  Find downloadable forms by visiting our Patient Resources page.  Please complete the patient registration form and bring it with you to your appointment, along with the following:

If you have insurance we ask that you bring your insurance card, and your copay if you have one.

  • If you would like to apply for a discount based on the sliding fee scale we ask that you bring proof of your income. Our sliding fee scale is offered to all patients based upon family/household size and annual income.
  • Records from your previous doctor’s office.
  • Immunization records

Refuah Health Center, Inc. continues to provide high quality health care in Rockland, Sullivan, Orange and Ulster Counties to all residents who need it, regardless of their ability to pay, however, for those patients who do have, or are eligible for, health insurance, the following is a list of the Insurance Plans Refuah Health Center accepts: Download the complete list of accepted plans
